Participating non-profits will be selected by a panel of judges based on their past good work and need after supplying qualifying information, including submission of the following:

• Application form (PDF)
• Copy of your Internal Revenue Service 501(c)(3) designation letter
• Copy of your 2010 financial statements
• List of Board of Directors
• Copy of your By Laws (if applicable)
• Acknowledge acceptance of Contract Terms and Conditions
